Sometimes, we need a bit of extra support, for example, when coping with a short-or long-term health situation, or when life throws other challenges our way. We help in areas such as mental health and wellbeing, general health and fitness, social isolation, housing and environment, debt, and finance.

The Community Wellbeing Team is here to offer support and advice to you during these times.

Our team is made up of Community Wellbeing Social Prescribers based at GP Surgeries across Wrexham and Community Agents who provide an outreach service within each community council area across Wrexham. They all have i excellent local knowledge and understanding of local communities and will offer you a face-to-face, person-centered service.
